  • Introduzione: L'innesto osseo al processo alveolare rappresenta il trattamento migliore per la correzione della sua schisi. Utilizzando metodiche radiologiche bidimensionali è impossibile una valutazione quantitativa dei risultati. L'introduzione della TC come-beam consente una valutazione effettiva dei risultati. Presentiamo una nuova classificazione anatomica basata sulle tecnologie 3D. Metodi: Abbiamo analizzato 29 pazienti con schisi al processo alveolare, non sindromici, mediante una TCCB subito prima e 6 mesi dopo l'intervento chirurgico. E' stata elaborata una ricostruzione 3D del difetto anatomico per ogni paziente. Questo ha consentito la misurazione esatta della quantità di osso necessaria durante l'intervento e la conformazione di un misurino a forma di cucchiaio per 2 volumi diversi: il "volume funzionale" corrispondente alla quantità di osso necessaria per ricreare il processo alveolare; il "volume morfologico" rappresentato dalla quantità necessaria a colmare il difetto fino all'apertura piriforme. infine è stato valutato il riassorbimento dell'osso innestato mediante un confronto tra la TC pre e post operatoria. Risultati: il 65,5% dei pazienti ha presentato un risultato soddisfacente, maggiore al 90% del volume funzionale. In 3 pazienti il volume ricostruito è stato simile a quello morfologico. Solo 1 paziente è stato un fallimento legato a un'infezione post-operatoria. Non sono state riportate altre complicanze. Conclusioni: L'introduzione della TCCB ha portato al chirurgo diversi vantaggi associati alla creazione di un modello tridimensionale del difetto anatomico. Consente una maggiore precisione nell'allestimento della tasca per l'innesto oltre alla stampa di un contenitore corrispondente alla quantità di osso necessaria. In questo modo è possibile minimizzare il danno a carico del sito ricevente; si può inoltre ottenere un'accurata valutazione del risultato e dell'entità del riassorbimento.
  • Background: Alveolar bone graft is the standard of care for alveolar cleft treatment. A quantitative assessment of the result is difficult using bi-dimensional imaging methods. The introduction of the cone-beam computed tomography (CBCT) allows an effective evaluation of the surgical outcome. We present a new classification system based on 3D anatomical modelling. Methods: We analysed 29 non syndromic cleft patients with CBCT immediately before the surgery and 6 months after surgery. A 3D reconstruction of the anatomical defect was obtained for each patient. This evaluation allowed an exact measurement of the quantity of bone needed during the surgery and the creation of a measuring spoon for two different volumes: the “functional volume”, corresponding to the quantity of bone needed to recreate the alveolar process; the “morphological volume”, represented by the bone needed to completely restore the defect up to the anterior nasal aperture. Finally, the bone graft resorption was evaluated through the comparison between the pre-operative and the post-operative CBCT. Result: 65,5% of the patients had a satisfactory result that was at least 90% of the volume defined as “functional”. In 3 patients, the volume obtained was similar to the “morphologic” one. Only 1 patient had poor results because of post-operative infection. No other complications were reported. Conclusions: The introduction of the CBCT gives to the surgeon several advantages other than recreate a 3D model of the anatomy of the bone defect. It permits to obtain a more suitable pocket for the bone graft and to print a measuring spoon corresponding to the volume of bone needed to fill the cleft. In this way, it is possible to minimize the trauma on the donor site and quantify the bone used during the operation in order to have an accurate evaluation of the outcomes and of the reabsorption rate.
